Clinical experience with a double-tracer test of the oral absorption of 58Co-vitamin B12 (free vitamin B12) and of 57Co-vitamin B12 with intrinsic factor (bound vitamin B12) is reported. Absorption of the free and bound vitamin was measured simultaneously using a whole-body monitor. Control subjects absorbed 19.6 to 84.9% of free vitamin B12 and the absorption ratio of bound/free in any subject did not exceed 2. In patients with pernicious anemia, the absorption of free vitamin B12 ranged from 0.3 to 16.2% and the absorption ratio in all patients was greater than 2. Technical sources of error are considered and examined; the errors are shown to be acceptable and the advantages of the test are outlined.
